T-LPN (Job ID OOJ - 7060)

Responsibilities:

• Provide direct patient care under the supervision of an RN or physician
• Monitor and record vital signs, patient conditions, and report any changes promptly
• Administer medications and treatments as prescribed, ensuring accuracy and safety
• Perform wound care, dressing changes, catheter care, and other clinical procedures
• Assist patients with activities of daily living (ADLs) such as bathing, dressing, and feeding
• Maintain accurate and timely documentation using electronic health record (EHR) systems
• Collect laboratory specimens, including blood draws, and perform basic diagnostic tests
• Monitor blood glucose levels and administer insulin as ordered
• Initiate and monitor IV fluids (as per state scope of practice)
• Provide patient and family education on medications, treatments, and care plans
• Collaborate with the healthcare team to ensure coordinated and effective patient care
• Respond to emergencies and provide immediate care as needed
• Follow infection control policies and maintain a safe, clean environment
